Abstract:

Objectives: To retrospectively evaluate fracture rates of monolithic and layered zirconia restorations up to 7.5 years.

Methods: Two dental laboratories with a data-base system capable to track the number of returned restorations due to fracture only, were calculated. Both laboratories provide a 5-year warranty for zirconia restorations. The first evaluation of monolithic and layered zirconia restorations fracture rate was according to restoration type: single restorations and fixed partial dentures. The second evaluation was according to restoration location: anterior and posterior. Restorations with poor fit, shade and marginal integrity were excluded from evaluation. Fracture rates were compared and analyzed with Chi-square test (α=.05).

Results: A total of 93,848 monolithic restorations, and 43,096 layered zirconia restorations were evaluated. Number of fractured units and percentage of fracture can be seen in Tables 1 and 2. Layered zirconia single restorations fractured at a significantly higher rate than monolithic zirconia single restorations. Monolithic zirconia fixed partial dentures fractured at a significantly higher rate than monolithic single restorations. Anterior monolithic zirconia restorations fractured at a higher rate than posterior monolithic zirconia restorations.

Conclusions: Within the limitations of this study, monolithic zirconia restorations fractured at a relatively low fracture rate up to 7.5 years. Layered zirconia restorations also fractured within acceptable limits, more than monolithic restorations. Randomized control clinical trials are required to confirm these evaluations.
